Transport Services
Coastal's Transportation Division started in 1991 with the successful award of the Medicaid transportation
contract. We have continued to provide wheelchair and ambulatory services since that time. Over the years
we have grown and now offer a variety of transportation services.
Services we currently provide:
Medicaid Wheelchair
Medicaid Ambulatory
Medicaid Non-Medical Stretcher
Private pay Wheelchair
Private pay Non-Medical Stretcher
Medicaid Transportation Service:
Coastal Health Systems has the Medicaid Transport Contract for Brevard County. These transportation
services are available for those persons with Medicaid benefits who meet the Medicaid criteria for
transportation benefits and have no other means of transportation.
In order to be eligible for Medicaid funded transportation you must meet the following criteria.
Current recipient of Medicaid Transportation benefits.
Must be going to seek treatment or medical services from a Medicaid approved provider.
Must be treated for a Medicaid approved service.
If you are not certain if you meet this criterion please contact your Medicaid Program Office or feel
free to speak with one of our Reservationists.
Medicaid Transportation Services: Available Monday-Saturday 6am-6pm.
How to reserve a trip and what to have ready:
To schedule a non-medical stretcher transport, call AmeriTrans at (321) 633-8505 or fax your request
to (321) 633-8501, 8am-8pm, 7 days a week. NOTE: Transports after 8pm may be arranged and provided
on a case by case basis. Medicare and most insurance companies do not cover non-medical stretcher
service. For comprehensive information regarding service fees, please contact the AmeriTrans
billing office at (321) 632-5092.
To schedule an Ambulance transport, call (321) 631-1448, or fax your request to (321) 633-8501, 24
hours a day, 7 days a week.
The Reservations Office is open Monday-Friday from 8:00am to 2:00pm. The telephone number is
(321) 632-7030. Transports require a minimum of three days advance notice to schedule your transport.
For Example: if you need a trip for Friday, you will need to call and reserve a trip no later than
2pm on Tuesday.
When you call to reserve a trip please have the destination address and your Medicaid number ready to
give to the Reservationists. You will also be required to give your appointment time, the reason why
you are seeking treatment or services, and the estimated time your appointment will be done.
Health Care Provider's requesting Medicaid transportation for their patients may call our Dispatch
office and request a fax sheet to be sent to you to complete and fax back for confirmation or email your questions to billquestions@coastalhealth.org
Coastal is a Shared Ride Program
Medicaid transportation is a shared ride program therefore you will most likely be riding with other
clients going to appointments at similar times or at similar locations. It is advisable to bring
along something to pass the time. A book, puzzlebook, etc. If you have special diet needs such as
diabetes or low blood sugar then bring a snack with you.
Once you have reserved your trip, you need to be ready to ride two hours before your appointment,
the driver will pick you up in a timely manner. For example: If you have an appointment at 11:00am,
you will be told to be ready to ride by 9:00am, it does not mean that the driver will be there at
9:00am, you simply need to be packed, ready to go, and expect the driver to arrive shortly.
Co-Pay
Medicaid requires Coastal to collect a one-dollar ($1) co-pay on each leg of the trip, unless you meet the
following criteria:
Under the age of 21
Receiving family planning or pregnancy related services
Resides in a Skilled Nursing home
Hospice patient
Please have your co-pay ready when you board the vehicle. At the end of the month all uncollected
co-pay will be calculated and remaining balances will be sent to you.
Escorts:
Escorts will be required for all riders under the age of 18 years. Escorts may accompany a rider if
the rider is in a wheelchair, is blind, or requires supervision or assistance before, during, or after
their appointment. Children not attending an appointment will not be permitted to ride.

We understand that on occasion you may need to miss your scheduled ride. Please make every effort
to contact us as soon as you know you can't make your ride. Those trips cancelled three days or
more in advance will not be penalized. Those trips canceling in less than three days will be marked
as a cancellation. Those trips canceling with less than twenty-four (24) hours notice or on the same day
will be marked as a no-show. All no-show and cancellations are reported to the Medicaid Program
office. Repeat no-shows and cancellations may result in suspension of transportation services.
